Who is Eligible?
Students 12+ yrs. old & their Family Members
Anyone 12 - 17 yrs. old can NOT be vaccinated without the signed parent consent form

Second dose will be administered at the same time and place on August 27th
Anyone needing a second dose of Pfizer COVID-19 vaccine is also eligible
