Belle Valley Elementary School issued the following announcement on Sept. 30.
All students in Kindergarten are required by the state to have a physical in their health record at school regardless if you are virtual or at school. In addition, all students that are virtual should turn a physical into the school nurse. Parents or guardians can have the doctor fax physical forms to the nurse at 814-835-5635 or mail them to the school.
All students in Kindergarten and 3rd grade are required by the state to have a dental exam in their health record at school. Even if the student is virtual, the state requires a dental exam in their health file. Please have the dentist fax the dental exam record to the nurse at 814-835-5635 or mail them to the school.
If you need a form for either of these they can be found on the Health Services page. It does not matter if it is on the MTSD form or the dentist/doctor office form. Either form will work.
